WHAT IS PROSTATE CANCER?

Prostate cancer originates in the prostate, a gland exclusive to men. About the size of a walnut, it is soft to the touch and situated at the intersection of the bladder, penis, and rectum. When prostate cells start to grow and multiply abnormally, the gland enlarges and exerts pressure on the urethra, the tube that carries urine from the bladder.

UNDERSTANDING THE CAUSES

Exposure to specific risk factors can lead to damage in the DNA and genes within prostate cells. These genes regulate the growth and multiplication of cells, ensuring that abnormal ones are eliminated. When these genes are damaged or lost, critical instructions are altered or deleted, resulting in chaotic growth of prostate cells—the hallmark of prostate cancer.

HOW COMMON IS PROSTATE CANCER?

Prostate cancer is far from rare. Following skin cancers, it stands as the most prevalent cancer among men. In the United States alone, over 3 million men were living with prostate cancer in 2018. Globally, the prevalence rate reached 4.9 million cases in 2020.

A WORLDWIDE CONCERN

Europe carries a significant burden, accounting for nearly 38% of global prevalence. However, prostate cancer has made its mark across the continents. It’s the most frequently diagnosed cancer in at least 112 out of 185 countries.


Compared to other cancers, low-risk prostate cancer is the most common cancer among European men and contributes significantly to new cancer cases.

HOTSPOTS OF PREVALENCE

Regions like Guadeloupe, Martinique, Ireland, and Barbados lead the world in prostate cancer prevalence rates. Though complex factors contribute, organochlorine pesticides are identified as possible contributors. In the US, Canada, and Australia, prostate cancer incidence grew at very uncomfortable rates in the 1990s and 2000s. 

NEW CASES AND SURVIVAL ODDS

In 2021, an estimated 248,530 new cases of prostate cancer will be diagnosed in the United States. Globally, there were 1,414,259 new cases in 2020. Survival rates are promising: the 5-year survival rate is 100% for localised or regionally spread cases. However, the rate decreases to 30% when cancer has reached distant organs.

RACIAL DISPARITIES

African-American men face higher risks of both diagnosis and mortality. Debates continue on whether this stems from genetics, access to healthcare, or socio-economic conditions.

SCREENING AND DIAGNOSIS

Screening and diagnosis in the 1990s and 2000s were pegged on Prostate-Specific Antigen (PSA) tests.

However, when prostate cancer screening and testing policy was changed—and PSA tests discouraged—the rates dropped significantly, coinciding with the changes in these policy recommendations.

IS THERE AN ISSUE WITH PSA TESTING?

PSA tests may reveal prostate cases that turn out to be false positives or harmless cases. In fact, according to the National Cancer Institute, only 25% of biopsies performed as a result of positive PSA tests, do actually reveal prostate cancers.
